We want to make your first visit in our office easy and enjoyable. For your convenience, please complete your new patient forms online. If you are utilizing insurance, please bring your current insurance card as well as a driver’s license or other form of photo identification with you to your first appointment. Your first appointment will be approximately 60 minutes as we do a thorough consult, exam and if needed x-rays to ensure you receive appropriate care.
